Research Overview
Zenagamtide — formerly known as amycretin, Novo Nordisk development code NN9487 — is an investigational unimolecular peptide that simultaneously activates the glucagon-like peptide-1 (GLP-1) receptor, the amylin receptor, and the calcitonin receptor. The compound's design addresses a structural property of the amylin receptor itself: the amylin receptor is a heterodimer formed by the calcitonin receptor paired with a receptor-activity-modifying protein (RAMP), which means that GLP-1 and amylin co-agonism in a single peptide chain inherently extends to calcitonin receptor activation. This places zenagamtide in a mechanistically distinct position from CagriSema, which achieves GLP-1/amylin co-activation by combining two separate molecules (cagrilintide + semaglutide). Phase 2 subcutaneous trial data published in The Lancet (August 15, 2026; PMID 42532080) showed once-weekly zenagamtide across a dose range of 0.4–40mg in adults with type 2 diabetes produced up to 14.6% body weight reduction and HbA1c reductions of up to 1.71 percentage points at 36 weeks, with 89.1% of participants achieving HbA1c below 7% at the highest dose. Earlier Phase 1b/2a data in adults with obesity without type 2 diabetes showed up to 24.3% mean body weight reduction at 36 weeks — a result exceeding approved GLP-1 monotherapy at comparable timepoints. Tolerability profile in both trials was consistent with other GLP-1-based and amylin-based therapies, predominantly gastrointestinal. A companion Phase 2 trial of an oral zenagamtide formulation was also published in The Lancet on the same date (PIIS0140-6736(26)01247-X). Novo Nordisk announced Phase 3 programs — AMAZE (obesity indication) and AMBITION (type 2 diabetes) — for initiation in H2 2026. Zenagamtide is not FDA approved and is not available outside authorised clinical trials as of August 2026. For research and educational purposes only. Not FDA-approved.
