Eloralintide: amylin selectivity enters Phase 3

By Peptide Hub Research Team · October 31, 2026 · 9 min read

Eloralintide (LY3841136) is Eli Lilly’s selective long-acting amylin receptor agonist—an amylin-pathway candidate that is not a GLP-1 and not a unimolecular GLP/amylin dual like amycretin. Phase 2 obesity data published in The Lancet showed dose-dependent weekly weight loss from roughly 9% to 20% at 48 weeks versus near-zero placebo change. Phase 3 monotherapy and combination programs (including with tirzepatide) are now the operational story. This profile is for researchers tracking the post-incretin pipeline.

Mechanism: AMY1R-preferring amylin agonism

Amylin is co-secreted with insulin and reduces energy intake via central and gastrointestinal satiety pathways. Eloralintide is engineered as a selective agonist with substantially higher potency at human AMY1R than at the calcitonin receptor, and it uses fatty-acid albumin binding to support once-weekly dosing. That selectivity story is why it is grouped with cagrilintide-class thinking rather than with pure GLP-1s.

Phase 2 weight-loss results

In NCT06230523, adults with obesity/overweight without T2D received weekly placebo or eloralintide at 1, 3, 6, or 9 mg, or escalation arms to 9 mg, for 48 weeks. Efficacy-estimand mean body-weight changes spanned about −9% (1 mg) to about −20% (9 mg and 6–9 mg) versus about −0.4% placebo. GI events were the dominant tolerability theme.

Phase 3 and combinations

Multiple Phase 3 trials are registered for obesity with and without T2D, osteoarthritis pain, OSA, and incretin combination settings. Sponsor disclosures on EloraTZP (eloralintide + tirzepatide) describe large mid-stage weight-loss increments over tirzepatide alone—still investigational.

How it differs from CagriSema and amycretin

CagriSema co-formulates cagrilintide (amylin) with semaglutide (GLP-1). Amycretin is a single molecule hitting GLP-1 and amylin together. Eloralintide is selective amylin agonism that can be stacked or combined strategically with incretins rather than baking GLP-1 into the same chain. Pipeline literacy starts with that taxonomy.

Regulatory status

Not FDA-approved. Not a compounded 503A bulk-substance story like the HHS six. Treat research vials and trial drug as distinct from approved obesity pharmacotherapy.
