Petrelintide (zp8396) is a long-acting human amylin analogue developed by zealand pharma in partnership with roche for chronic weight management. amylin is co-secreted with insulin from pancreatic beta cells and signals through amylin receptors in the hypothalamus and brainstem to reduce food intake, slow gastric emptying, and suppress glucagon — pathways independent of glp-1 and gip receptor agonism. this mechanistic distinction positions petrelintide as a potential alternative or complement to incretin-based obesity therapies, with particular interest in gastrointestinal tolerability. the phase 2 zupreme-1 trial (nct06662539; n=485 adults with obesity or overweight plus comorbidity; mean bmi 36.7) randomized participants to once-weekly subcutaneous petrelintide at 1.0, 2.5, 5.0, 7.0, or 9.0 mg or placebo for 42 weeks. full results published in the lancet diabetes & endocrinology (september 29, 2026) reported mean body-weight reductions of 7.9–9.8% at week 28 (efficacy estimand) versus 1.7% with placebo, with the 5.0 mg dose delivering the highest week-28 loss (−9.8%); weight continued to decline through week 42 (up to −10.7%). nausea was the most common adverse event (~20% vs 6% placebo); vomiting was infrequent. a global phase 3 zupreme registrational program was initiated in late september 2026 across obesity, obesity-plus-t2d, and obesity-plus-cvd populations. petrelintide is investigational and not fda approved. for research and educational purposes only. not fda-approved. consult a qualified healthcare professional.
