Cagrilintide is a long-acting amylin analogue — amylin is a peptide hormone co-secreted with insulin by pancreatic beta cells. it binds amylin receptors in the hypothalamus and brainstem to reduce food intake, slow gastric emptying, and suppress glucagon. its pharmacological half-life allows once-weekly dosing. cagrilintide is most extensively studied in combination with semaglutide (the combination is called cagrisema, developed by novo nordisk). phase 3 redefine-1 trial results showed cagrisema produced approximately 20.4% weight loss — greater than either agent alone — with a new drug application submitted to the fda in december 2025. when added to existing glp-1 protocols, cagrilintide may allow glp-1 dose reduction while maintaining or enhancing weight loss outcomes, potentially reducing dose-dependent nausea. research also suggests amylin receptor activation may independently improve insulin sensitivity beyond its appetite effects. for research and educational purposes only.
