Ara-290 (cibinetide) is a non-hematopoietic erythropoietin (epo) analogue engineered to remove epo's erythropoietic (red blood cell-stimulating) activity while preserving its tissue-protective and anti-inflammatory signaling through the innate repair receptor (irr). the irr is a heterodimer of the epo receptor (epor) and the common beta receptor (cd131) expressed on immune cells, neurons, and injured tissues — distinct from the classical epor homodimer that drives red blood cell production. ara-290 binding to the irr activates anti-inflammatory pathways including inhibition of nf-kb, reduction of pro-inflammatory cytokines (il-1beta, tnf-alpha, il-6), and stimulation of tissue repair signaling without thrombotic or erythropoietic side effects. fda fast-track designation was granted for autoimmune small fiber neuropathy (sfn) — a painful peripheral nerve condition — following phase 2b clinical trials demonstrating improved corneal nerve fiber density (a proxy for sfn severity) and symptom reduction. ara-290 is studied in the mcas protocol stack alongside vip (mast cell stabilization), kpv (mucosal anti-inflammation), and ta-1 (t-regulatory normalization). for research and educational purposes only.
